You may need official documents from your home institution (that’s us!) for a variety of reasons after you graduate from UF College of Medicine. Applying to a residency or fellowship program?

Congratulations on taking the next step in your professional career! The UF COM Office of Student Affairs is committed to the academic, personal and professional development of both current students and graduates. Our team is well-equipped to support you as you pursue lifelong learning through graduate medical education.

If you are a graduate applying to residency programs through ERAS, you will need to request a token to register for myERAS. The new application cycle begins mid-June of each year. Please send your email from the address you would like to receive the token.

If you are a graduate applying to fellowship programs, you will request a myERAS token through the ERAS Fellowships Documents Office (EFDO), a program overseen by the AAMC. Once registered, you will likely need your MSPE and your medical school transcript. There are three steps in this process after you receive your EFDO token:

  1. Email Atia Khan to notify our office of your fellowship applications and enter our document request queue. If you have official copies of your medical school transcript, you may not need to order a new copy or submit a MIDUS document request for your transcript.

Once you are in the queue, in any order,

  1. Submit a MIDUS document request for each document to be uploaded on your behalf.
  2. Order your medical school transcript. The Office of Student Affairs cannot request official transcripts from the University of Florida; transcript requests must be made by the graduate. Please read the following Transcript Request Guide to ensure it is delivered to the correct recipient.
